ACKNOWLEDGEMENT OF RISK, INJURY, AND OBLIGATIONS

I acknowledge that the activities I (or my child) have voluntarily agreed to undertake involve inherent risks and may be dangerous. By participating, I understand that I (or my child) am exposed to potential risks, including but not limited to physical, emotional, or mental injury.

Activities Covered Include (but are not limited to):

• Striking and grappling

• Cardiovascular exercise

• Strength and conditioning training

• Resistance training

• Personal training sessions

• Training activities both on and off the premises

• Any other services provided by 180 MMA Limited

Medical Disclosure

I (the participant/parent or guardian of the participant) confirm that I have disclosed any relevant past or present medical conditions that may impact participation in the above activities. I understand that it is my responsibility to inform a staff representative prior to commencing any training.

Venue-Specific Safety Precautions

• I understand that the 180 MMA gym venue has a mezzanine level that is out of bounds for any children 12 years old or younger, unless they are specifically invited upstairs and are accompanied and supervised by a 180 MMA coach.

• I agree to use all fitness equipment only for its intended purpose and to follow the safety instructions of trainers and managers at all times in order to avoid injury or property damage.

Acknowledgements

I understand and agree that:

• Participation in these activities is undertaken at my (or my child's) own risk.

• I (or my child) may be physically, emotionally, or mentally injured as a result of participating in the activity.

• 180 MMA Limited and its staff are not liable for any injuries sustained during participation.

• Personal property may be lost, damaged, or stolen during participation.

• Injury or property damage may occur due to the actions of other participants or my (or my child's) own actions.

• The conditions under which the activities are conducted may change without warning.

• I have been informed of and understand the safety precautions in place for the venue.

• Photos or videos taken during training sessions may be used for promotional purposes by 180 MMA Limited.

• Participation in on-site training includes the potential risk of exposure to viruses or other contagious illnesses, which I accept knowingly and willingly.

Payment Terms:

• The agreed-upon membership fee ($170.00 incl. GST) will be paid in full upfront via bank transfer to: 180 MMA Limited  01-0666-0326496-00
  Prompt payment reserves my child's spot in the chosen class. 

• I acknowledge that this is a fixed-term membership, which means it will automatically terminate at the end of Term 4, 2025.

Membership Cancellation:

• This membership will cancel automatically at the end of the agreed term.

• Early cancellation: The membership fee, whether in full or partial, is non-refundable.
